1.   A certified copy of the Birth Certificate
2.  Immunization Records

4.  Two (2) documents proving your address (utility bills, signed lease/rent agreement, W-2 forms)
5.  Health Assessment form filled out by doctor (for students entering Kindergarten)
2008-2009 School Year
If you will be new to Oakwood for the 2008-2009 school year, please contact the office to let us know.  We will be holding our Kindergarten Beginner's Day on Tuesday, May 6th at 3:30 and want to make sure that we have you on our list.  When you contact the school please have the following information:  Your name, child's name, child's date of birth, grade child will be in (very important if your child is not entering as a kinder), and your address.  We look forward to hearing from you and meeting you soon.
