5th Grade Supply List
• Pencils • Pencil Erasers Class Use: • Red ink pens for checking Boxes of tissues • Loose-leaf notebook paper Hand Sanitizer • Plastic or wooden 12”/30 cm ruler Sanitizing Wipes • Clipboard • One – 1½” plastic binder • Zipper Pencil Pouch • Dividers Book bags on wheels are discouraged. They cause problems when going up steps, on the bus, and are too large for cubbies. Please use backpacks.

4th Grade Supply List

Items to bring when registering your child
1. A certified copy of the Birth Certificate 2. Immunization Records 3. Social Security Card 4. Two (2) documents proving your address (utility bills, signed lease/rent agreement, W-2 forms) 5. Health Assessment form filled out by doctor (for students entering Kindergarten)

2008-2009 School Year
If you will be new to Oakwood for the 2008-2009 school year, please contact the office to let us know. We will be holding our Kindergarten Beginner's Day on Tuesday, May 6th at 3:30 and want to make sure that we have you on our list. When you contact the school please have the following information: Your name, child's name, child's date of birth, grade child will be in (very important if your child is not entering as a kinder), and your address.
