Offices-Southwest Elementary

To enroll your student at Southwest Elementary School you need to live in the Southwest District. If your child is coming from another country, you will need to visit our Welcome Center before enrollment. If your student has been in another school we will request school records. Please bring the following items to the school office with you.

Proof of Address (two forms)
Shot Records
Birth Certificate
Social Security Card

When you get to the Southwest Office you will be expected to fill out the following forms:

Student Registration
Home Language Survey
Student Medical Form
Parental Authorization for Field Trips
Insurance Information
Volunteer Information
Optional: Application for Free and Reduced Meal Prices
